College Advising – Parent Questionnaire College Advising - Parent Questionnaire Please submit your responses below by Friday, October 1st. Your child’s paperwork will be due Tuesday, October 5th. Once all documents from parent and student have been received, Megan Harper will schedule your family conference. This conference will include your family, your child’s grade chair, and college advisor. Conferences will be scheduled beginning in January based on the order the paperwork was received. Thank you for your time and attention to this form. Please assist us in learning as much about your daughter or son as possible by sharing information that might be useful in advising your child and writing a letter of recommendation to colleges. Your comments are extremely valuable in adding insights to our knowledge of your child; often, although we know your child in context of the school, there may be important things we do not know. Please use this information sheet as an opportunity to share your views with us. We appreciate the time that it will take to answer these questions and look forward to reading your responses. The questions below will address the following: • character traits, life experiences, passions/interests which make your child unique • strengths and weaknesses as seen from your perspective • reactions to special circumstances, challenges, or adversity • community and volunteer activities, travel/summer experiences, part-time work • personal accomplishments which may not be reported • motivation, initiative, involvement, and interpersonal skills Please provide a full response to the questions asked. Feel free to change the space allowed for answers. A brief story, anecdote, or incident that conveys character traits or judgments is MOST helpful. Please concentrate on your child’s high school years.Student's Name* First Last Parent completing the form:* First Last Parent's Email* Enter Email Confirm Email Father's InformationFather's Name First Last Father's Email Enter Email Confirm Email Attended college?
